Biolar Hemiarthroplasty Using Cement for the Treatment of Intertrochanteric Femoral Fracture in Elders -2 to 5 Year Results-

Abstract
Purpose: A study was undertaken to evaluate for a minimum of two years the results of treatment of comminuted intertrochanteric fractures in elderly patients with osteoporosis treated by bipolar hemiarthroplasty with a modern cementing technique.

Materials and Methods: Thirty-five cemented bipolar hemiarthroplasties were performed in patients greater than 70 years old who had comminuted intgerochanteric fractures. The mean age at surgery was 78 years old. All patients were followed for 3.8 years on average. We evaluated clinical and radiographic results.

Results: Most patients displayed excellent or good results and thirty patients regained their preoperative ambulatory levels. The mean Harris hip score improved to 85.6 and the mean time at full weight bearing following surgery was 5.6 weeks. Thirty patients regained fracture healing. There were no cases of osteolysis around the stem, no measurable subsidence of the stem, and no acetabular erosion. The incidence of radiolucency around the stem incrased with time. There was one case of possible loosning.

Conclusion: According to short term follow up, bipolar hemiarthroplasty combined with a good pressurized cementing technique is an effective option for the treatment of comminuted intertrochanteric fractures in elderly patients with osteoporosis. However, the increased incidence of radiolucency around the stem over time may be worrisome even though excellent fracture healing was achieved. Therefore, long term follow up is necessary to verify these preliminary
findings.
